I have to disagree with you on two counts:

first: Besides
The correct answer in this context is 'Beside' not 'Besides'.
According to the dictionary and in relation to the example you are asking about: Beside is a preposition which means: (among other things): apart from.

So in your example if we replace 'Beside' with 'apart from' we would say: "Apart from" the fact that it was difficult, the exam ....

Which is quite legitimate.

However, 'Besides' is an adverb which means: moreover. So, in your particular example, if we replace your word 'Besides' with "moreover", we will get the following:
"Moreover" the fact that it was difficult, the exam....

which is not a sound English sentence.

In the usages of the two words 'Beside' vs 'Besides' I have given you the most common practice followed in the English-speaking community. Having said that, there are instances where either can be used in your sentence. Which means you cannot dismiss one word over the other. I mean you cannot say, outright, that the use of the word 'Beside' is wrong in this context.

On the contrary, I would venture to say that the word 'Beside' is more correct than 'Besides' in your particular sentence.

Second: Headline Vs Title

The most common usage of the word 'headline' is as follows: The most important items (ie headlines) of the news very briefly stated. As in the sentence: here again are the headlines.

As for 'title' it simply means the heading of any article.

If you check the Oxford dictionary you will find that : Beside (prep) means: at the side of, or next to sb/sth
وهنا تعني : قرب / جانب
ie: Come and sit beside me.
He kept his bag close beside him at all times.

وتعني : علاوة على / بالاضافة الى While Besides : (prep) in addition to or as well as sb/ sth:
There will be six people coming , besides you and David.
notice Besides (adv) in addition ; also :
I thought the hotel was too expensive. Besides, it was very close to the main road.

AS you can see in the second part of the sentence there is the word (also) which confirms the meaning of Besides

the fact that it was difficult, the exam also included questions that we had never studied before __________

As for Headline and Title

According to Oxford dictionary :

Title: (noun) the name of a book, play, film, picture, etc.

Headline : (noun) the title of a newspaper article printed in large letters above the story
عنوان رئيسي في صحيفة

and as you can see in the sentence there is the word (article) which illustrates the meaning

" The article's _____ read "Politician Wants Answer
